Home Care After a Hospital Stay

Practical help in the days and weeks after discharge, for as long as it is genuinely needed.

Is this the right support?

Coming home from hospital is often harder than people expect. Someone can be medically ready for discharge and still find that the first fortnight at home is exhausting — stairs feel steeper, meals get skipped, and confidence takes longer to return than strength does.

Discharge support is short-term, practical care that covers that gap. Sometimes it is needed for two weeks; sometimes it becomes ongoing care. Both are fine, and you are not committing to the second by choosing the first.

The first fortnight matters

Most of the difficulty is concentrated into the first couple of weeks. Support at that point does more good than support started later.

Short-term is genuinely fine

You can arrange care for a defined period. If it turns out you no longer need it, that is a good outcome, not a lost sale.

We are based at Bretby Business Park, between Burton and Swadlincote. If a discharge date moves, we will tell you honestly whether we can cover the new date rather than agree first and check afterwards.

Being clear about what we do

Queen's Hospital Burton is the main hospital for many of the families we speak to, and we regularly support people returning home from a stay there. To be clear: we are an independent home care provider. We have no formal partnership, contract or affiliation with the NHS, the hospital or the local council.

If a discharge is being planned, the ward team or discharge coordinator can tell you what support has been assessed as needed. You are free to arrange private home care alongside that at any point.

Questions about hospital discharge support

How quickly can care start after discharge?

It depends on our availability at the time, so please call us as early as you can — ideally before the discharge date is confirmed. We would rather give you a straight answer about a specific week than publish a promise we cannot always keep.

Can we arrange care for just two weeks?

Yes. Short-term support is a normal reason to use us, and there is no setup fee and no minimum term to sign up to.

What if needs change once they are home?

The care plan is reviewed as things change. Visits can be increased if recovery is slower than hoped, or reduced and stopped as confidence returns.

Do you liaise with the hospital or the GP?

We work around whatever has been arranged and keep clear records of our visits. We are not part of the NHS discharge process and would not claim to be, but we are used to fitting in alongside it.
